OSF HealthCare providers are qualified to perform sports physicals for youth. Our providers
will conduct an exam and recommend additional tests and diagnosis if necessary. Following
the session, your OSF provider will fill in the required medical forms for the athlete. If your
child is going into 6th or 9th grade, they will need to see their Primary Care provider, due to vaccination requirements.
* Patients will be directed to their Primary Care Provider for their Return to Sports Physical if they have been diagnosed or suspected of having Covid-19 in the last 6 months.
