physicals and immunization

Does your child need a physical for school this year?  If they are new to our District or have never been in school before, they will need a physical and proof of immunization record.  Also any student entering  Kindergarten, 6th or 9th grade needs a physical/immunization record.  In addition, Seniors are required to show proof of the Meningococcal vaccine after the age of 16 years old.   The deadline for physicals and immunization records for all of these grades is September 15th.   Any student that doesn't provide the District with these documents by September 15th may be excluded from school.

 Lastly, any student that plans to play sports this school year will need an up-to-date physical on file to begin practicing/participating (this one can be a school physical or just a sports physical if they are not entering a grade that requires a school physical).   


Below are a couple of locations that are offering school physicals this summer if you are interested:

OSF Medical Group Pediatrics in Bloomington/Normal area will provide school and sports physicals on a walk-in basis during the first two weeks of August.

Walk-in hours will be:

·       August 2-6, 8 a.m. – 4 p.m.

·       August 9-13, 8 a.m. – noon
